Forms

Here you will find the appropriate forms you need to create a new site or make changes to your existing web site. Before you download any of the forms you will need Adobe Acrobat Reader. This will allow you to view and print the files from a separate window.

New Site Processing Form

For those just getting started this is the first form to download.

Domains (this is the wwww.sitename.com)
  1. Download the domain name request form
    We'll register your domain name use on ChoiceMall.
  2. Download the domain Transfer Form
    If you already own a domain and would like us to host it for you, fill out this form so we can start the transfer process.
  3. Download the domain Renewal Form
    Is your domain name about to expire? Renew it now to ensure someone else doesn't pick it up.
FTP Access Form

Already know HTML and would like to build and maintain a site yourself? Fill out this FTP Access Form and we'll set you up with an FTP account so you can make changes to your site whenever you want.

Deactivation Form

Need to take your site down for a period of time? Download the Web Site Deactivation form. We'll have your site down in 2 business days of receiving this form. We'll archive your site for when you're ready to re-activate.

Re-Activation Form

When you are ready to reopen your store, download the Web Site Re-Activation form. Your site will be back on the mall within 2 business days of receiving this request.
